Why Some Kids Get Cavities Even When They Brush
If your child brushes their teeth every day and still comes home from the dentist with a cavity, you're not doing anything wrong, and you're definitely not alone. Cavities remain the most common chronic disease of childhood in the United States, with more than half of kids ages 6 to 8 having had a cavity in at least one baby tooth. Brushing is an essential piece of the puzzle, but it's rarely the whole picture. Here's a look at what else might be going on.

Brushing Doesn't Always Reach Everywhere
Even a dedicated brusher can miss spots. Back molars have deep pits and grooves where a toothbrush bristle simply can't reach, and plaque that hides there has plenty of time to do damage. This is exactly why the American Dental Association points to dental sealants as one of the most effective tools for protecting those hard-to-clean chewing surfaces, since a sealant physically closes off the grooves before decay-causing bacteria can settle in.
Age plays a role here too. Most kids don't have the fine motor skills to brush thoroughly on their own until around age 10, which is why we, and the American Academy of Pediatric Dentistry, recommend that parents floss and brush for younger children and continue supervising until they've mastered the technique.
Snacking and Sipping Add Up
It isn't only how often your child brushes, it's how often their teeth are exposed to sugar and starch throughout the day. Every time a child eats or drinks something with fermentable carbohydrates, the bacteria in plaque produce acid that attacks enamel for roughly 20 minutes afterward. Grazing on snacks, sipping juice throughout the day, or falling asleep with a bottle keeps that acid cycle going almost continuously, giving teeth little chance to recover between meals. The CDC notes that fluoride varnish can prevent about one-third of cavities in baby teeth, which is part of why professional fluoride treatments matter alongside good snack habits at home.

Fluoride Exposure Varies
Fluoride strengthens enamel and can even reverse very early decay by remineralizing the enamel before a cavity fully forms, but not every child gets enough of it. Community water fluoridation reduces cavities by roughly 25%, according to the CDC's research on water fluoridation levels, yet millions of households rely on well water or filtered water that isn't fluoridated. Add in inconsistent use of fluoride toothpaste, and a child can be brushing diligently while still missing a key layer of protection.
Genetics and Enamel Strength
Some children are simply born with a disadvantage. Enamel thickness and mineralization are influenced by genetics, and conditions like hypomineralized enamel leave teeth more porous and more vulnerable to acid, no matter how carefully they're cleaned. Saliva composition is inherited too, and saliva plays a major role in neutralizing acid and washing away food particles. Two children with identical brushing routines can end up with very different cavity risk simply because of the biology they were born with.
Shared Bacteria
The bacteria most responsible for cavities, Streptococcus mutans, can be passed from caregiver to child through everyday contact: sharing a spoon, testing food before feeding a baby, or cleaning off a dropped pacifier with a parent's mouth. A child can pick up a higher bacterial load well before they're even old enough to hold a toothbrush.
Mouth Breathing and Dry Mouth
Saliva is one of the body's best natural defenses against cavities, and children who breathe primarily through their mouths, often due to allergies, enlarged tonsils, tethered oral tissues or airway issues, tend to have drier mouths with less saliva available to neutralize acid and remineralize enamel. This is one of the more overlooked risk factors, and it's one reason a comprehensive evaluation sometimes uncovers more than a dental exam alone would.
